[ ] I agree to follow reasonable safety instructions provided by the guide and any third-party activity operators.
[ ] I understand that I am responsible for carrying and managing my personal medications and advising the guide if my ability to participate safely changes.
[ ] I confirm that the information provided is accurate to the best of my knowledge and agree to advise the organiser of any relevant changes before or during the retreat.
[ ] In the event of an emergency, I authorise the organiser to contact emergency services, provide relevant information to medical personnel and notify my nominated emergency contact where appropriate. I understand that medical treatment decisions remain subject to applicable law.

The information collected on this form will be used by Flying Parrot Photography to assess participation needs, plan appropriate safety measures and respond to emergencies.
Relevant information may be shared with emergency services, medical personnel or activity operators where necessary for safety and as permitted by law.
Information will be stored securely, access will be restricted to those who need it for these purposes, and it will be retained and disposed of in accordance with applicable privacy and recordkeeping requirements.
